We would like to know what you think about the Air Quality Health Index [AQHI], and how you use it to manage your daily activities.

You may wish to complete the survey after you have had the opportunity to use the AQHI over a period of time.  Please note that you can complete the survey only once. 

Please respond to all questions as completely and accurately as possible. The survey will take about 5 minutes to complete.  We do not ask for your name so all responses will be anonymous.
